The DyDo Group (hereinafter, the "Group") discloses the following pursuant to the Act on the Protection of Personal Information (hereinafter, the "Act").
The following information includes matters that are required by the Act to be put in "a form readily accessible to the subject person" and "a form accessible to the subject person (including cases where a reply is given without delay at the request of the person)."

1. Purpose of Use
(1) In the case where personal information is acquired directly from the subject person in writing (including through a website or email)

The purpose of use will be clearly stated each time unless it is urgently necessary for the protection of the life, body, or property of a person or unless otherwise permitted by the Act.

(2) In the case of other than (1) above 

Except as permitted by the Act, the information will be handled within the scope of the following purposes of use.

For the purposes of use that have been individually notified, disclosed or given consent, in addition to the above, the information will be used appropriately within their scope. Phone calls received by Customer Service or the like may be recorded for an appropriate response.

(3) Use of statistically processed data

All personal information may be used for the creation, analysis or disclosure of statistical materials after deleting any personally identifiable items.

2. Joint Use

"Joint use" is a system permitted by the Act in which personal data (personal information systematically structured so that it can be easily searched) can be used jointly with a specific party by notifying certain matters in advance. The Group may use the information as follows, but will not jointly use it beyond the scope necessary to achieve its purpose of use.

(1) Items of personal information to be jointly used

Name, postal code, address, telephone number, age, date of birth, gender, occupation, email address, and other necessary information

(2) Scope of parties who jointly use personal information

Consolidated companies and equity-method affiliates of DyDo Group Holdings, Inc. and their sales partners

(3) Purpose of joint use

The information will be used within the scope specified in "1. Purpose of Use" above.

3. Purpose of Use of Retained Personal Data

"Retained personal data" shall mean personal data for which the Group has the authority to disclose, correct, add, delete, suspend use, erase, or suspend provision to a third party.
The purpose of use of the retained personal data held by the Group shall be as stated in "1. Purpose of Use" above, except where permitted by the Act.

4. Provision to a Third Party

The Group will not provide personal information to any third party other than the party to whom its business is entrusted without the prior consent of the subject person, except where permitted by the Act. (Regarding joint use of personal information, please see "2. Joint Use" above.)

5. Security Control Measures

The following are the security control measures for retained personal data by the Group.

(1) Formulation of basic policy

In order to ensure the appropriate handling of retained personal data, the Group has formulated internal rules and guidelines concerning the system for handling retained personal data, and its management, inspection, collection, use, storage, and disposal.

(2) Organizational security control measures

The Group has appointed a person responsible for, and a person in charge of, handling retained personal data to check the handling status, and holds periodic report meetings on information security, including personal information. The Group also has a system in place for responding to cases where information leakage or other issues are detected.

(3) Personnel security control measures

The Group obtains a written pledge for the handling of personal information from employees, and regularly provides them with training on the handling of retained personal data.

(4) Physical security control measures

The Group takes measures to prevent theft or loss of retained personal data by such means as managing the areas where retained personal data is handled through access control using IC cards, installation of partitions and seating arrangements, as well as storing the equipment that handles retained personal data, and electronic media or documents on which retained personal data is recorded in a lockable cabinet.

(5) Technical security control measures

The Group manages and restricts access rights to retained personal data according to job duties, and has introduced security software into information systems and equipment to protect them against unauthorized access from the outside and unauthorized software.

(6) Understanding the external environment

The Group implements security control measures as described above based on its understanding of the systems for the protection of personal information in foreign countries where retained personal data is stored.

6. Procedures for Disclosure, Correction, Suspension of Use, etc.

Upon request from a subject person, the Group will, in principle, respond appropriately without delay as follows. If we are unable to respond to the request, we will notify the subject person thereof. Personal data provided to the Group through entrusted business is not subject to disclosure, etc. because the Group does not have the authority to disclose, etc. such data.

(1) Notification of the purpose of use of retained personal data

Upon request from a subject person for notification of the purpose of use of retained personal data, the Group will notify said purpose of use by the method prescribed by the Group, only when the identity of the subject person can be confirmed. However, please note that we may not be able to give notification if any of the following situations apply.

  • 1) When notifying the subject person of the purpose of use or making it public may harm the life, body, property, or other rights or interests of the subject person or a third party
  • 2) When notifying the subject person of the purpose of use or making it public may harm the rights or legitimate interests of the Group
  • 3) When it is necessary to cooperate with a national agency or a local government in executing the affairs stipulated by law, and when notifying the subject person of the purpose of use or making it public may hinder the execution of said affairs
  • 4) When the purpose of use of the retained personal data related to the subject person is clear from the circumstances of the acquisition or these Disclosed Matters
  • 5) When the identity of the subject person cannot be confirmed, such as when the name or address on the application form does not match that on the identification documents
  • 6) When the request is made by a representative and the right of representation cannot be confirmed
  • 7) When other documents required for the procedures are incomplete
  • 8) When the storage period has expired and the data has already been disposed of
(2) Disclosure of records related to the provision of retained personal data or personal data to a third party

Upon request from a subject person for disclosure, the Group will, in principle, disclose said information by the method specified by the subject person only when the identity of the subject person can be confirmed. However, please note that we may not be able to disclose such information if any of the following situations apply.

  • 1) When disclosure may harm the life, body, property, or other rights or interests of the subject person or a third party
  • 2) When disclosure may significantly hinder the proper implementation of the Group’s business
  • 3) When disclosure violates laws and regulations or when otherwise provided for in laws and regulations
  • 4) When the identity of the subject person cannot be confirmed, such as when the name or address on the application form does not match that on the identification documents
  • 5) When the request is made by a representative and the right of representation cannot be confirmed
  • 6) When other documents required for the procedures are incomplete
  • 7) When the storage period has expired and the data has already been disposed of
(3) Correction, addition, or deletion (hereinafter, "Correction, etc.") of retained personal data

Upon request from a subject person, if there are any errors or changes in the information about him/her, the Group will perform Correction, etc. of said information and notify the details thereof by the method prescribed by the Group only when the identity of the subject person can be confirmed. However, please note that we may not be able to perform Correction, etc. if Correction, etc. is not necessary in light of the purpose of use or if what is pointed out in the request is not correct.

(4) Suspension of use, deletion, or suspension of provision to a third party (hereinafter, "Suspension of Use, etc.") of retained personal data

Upon request from a subject person for Suspension of Use. etc., the Group will perform Suspension of Use, etc. and notify the subject person thereof by the method prescribed by the Group only when the identity of the subject person can be confirmed. However, please note that we may not be able to perform Suspension of Use, etc. in the following cases.

  • 1) When Suspension of Use, etc. is requested for any reason other than the following
    • ・Where there is a violation of limitation of handling of personal information by purpose of use, prohibition of inappropriate use, proper acquisition, or restrictions on provision to third parties
    • ・Where it is no longer necessary for the Group to use such data
    • ・Where there arises leakage, loss, damage, or other circumstances that affect the security of retained personal data and that are highly likely to harm the rights or interests of the subject person
    • ・Where the rights or legitimate interests of the subject person may be harmed
    • ・In other cases as provided by the Act
  • 2) When what is pointed out in the request is incorrect
  • 3) When it is difficult to perform Suspension of Use, etc., and alternative measures are taken as necessary to protect the rights or interests of the subject person
<Procedures>
  • (1) The applicant should download the application form prescribed by the Group from the following according to the application content.

  • (2) Fill in the application form and send the following documents and others by mail to the address specified by the Group. Postage charges should be borne by the applicant. Please note that the sent documents and others will not be returned.

    • 1) Application form
    • 2) A document showing the identity of the subject person*1
    • 3) A reply envelope (839 yen worth of postage stamps should be attached [breakdown: standard size mail fee: 84 yen (up to 25 g), registered mail fee: 435 yen, certified mail fee: 320 yen]*2)
    • *1 Please enclose a copy of the portion of any one of the following documents that shows the name and address of the subject person registered in the Group. If the information on the documents is different from that registered, an official certificate that shows the history of changes should be submitted. In the case of a driver's license, passport, or other document that includes the registered domicile of the applicant, please make sure that such information is excluded from the information provided.

      The above documents will not be used for any purpose other than confirming identity, and will be disposed of without delay after the response to each request is completed.

    • *2 If the amount is insufficient or stamps are not enclosed, notification to that effect will be given. However, please note that if payment is not made within the specified period, the request will be treated as if it had not been made.

  • (3) In case of request by a representative, the following should be sent in addition to the above. Please note that these documents also cannot be returned.

    • 1) Letter of attorney from the subject person

    • 3) A document showing the identity of the representative (see *1 above)
      If the representative has the right of legal representation, a copy of a document certifying the right of legal representation (e.g., certificate of family register, certificate of registered matters) should be sent instead of 1) and 2) above.
